Home Mobility Assistance

For many patients, returning home after illness, surgery, or hospitalization can be just as challenging as the medical recovery itself. One of the most common barriers to safe discharge and independent living is the lack of appropriate mobility equipment.

At All Things Home Care, we believe that every individual deserves the opportunity to return home safely and with dignity. Through our Home Mobility Assistance program, we help bridge the gap between medical care and community reintegration by providing essential mobility equipment to those in need.

What We Provide

Through generous donations and community support, we are able to supply a variety of assistive devices, including but not limited to:

  • Chair lifts
  • Walkers and rollators
  • Wheelchairs
  • Hospital bed
  • Shower chairs and transfer benches
  • Other mobility and home safety equipment as available

These items are provided to patients who would otherwise face delays or barriers in being discharged from care and returning to their homes and communities.

Insurance and Coverage

In some cases, mobility equipment may be covered by insurance, Medicare, or Medicaid. However, coverage is not always guaranteed, and many patients face financial or logistical challenges in obtaining what they need in a timely manner.

Our program steps in to help when coverage is unavailable, delayed, or insufficient—ensuring that lack of equipment does not prevent someone from going home.

Who Is Eligible?

Home Mobility Assistance is available to individuals who:

  • Are preparing for discharge from a hospital, rehabilitation facility, or skilled nursing facility
  • Require mobility or safety equipment to return home
  • Have limited access to necessary equipment through insurance or other means

Referrals may come from healthcare providers, social workers, case managers, or family members.
